Important Disclosure: Mobile opt-in and information obtained as part of the SMS consent process or numbers for the purpose of SMS are not shared with any third parties or affiliates for marketing or any other purpose.
By opting in to SMS notifications from Be Well LifeStyle Centers, you consent to receive text messages related to your care, account, and our services. Your consent is voluntary and is not a condition of purchasing any goods or services from Be Well LifeStyle Centers.
Types of SMS Communications
You may receive the following types of text messages from Be Well LifeStyle Centers:
Customer Care
Messages related to appointment scheduling, follow-ups, and patient support.

"Hello, this is Be Well LifeStyle Centers. You had an appointment inquiry earlier today. Would you like to schedule a visit? Reply STOP to opt-out of SMS messaging. Message and data rates may apply."
Account Information
Messages related to your patient account, appointment confirmations, billing reminders, and wellness assessment results.
Example:
"Your account has been activated with Be Well LifeStyle Centers. You can now book appointments at bewelllifestylecenters.com. Reply STOP to opt-out."
Marketing
Promotional messages about wellness packages, new services, special offers, and health tips.
Example:
"Be Well LifeStyle Centers is offering special wellness packages this month. Visit bewelllifestylecenters.com for details. Reply STOP to opt-out."
How to Opt In
You may opt in to receive SMS notifications from Be Well LifeStyle Centers through any of the following methods:
- Website form — by submitting your mobile number and checking the SMS consent box on any form at bewelllifestylecenters.com
- During patient intake — by checking the SMS consent checkbox on any intake or consent form at our offices
- Verbal consent at the front desk — by verbally requesting to receive text message notifications during check-in at any of our locations (Birmingham, Berkley, or UWM)
- Online check-in — by providing your mobile number and consenting to notifications during the online check-in process
- Texting JOIN — by texting the keyword JOIN to our business number
Your mobile phone number on file in our patient management system will be used for SMS delivery. You must provide a valid mobile phone number capable of receiving text messages.
Message Frequency
Message Frequency May Vary. Our SMS message frequency will be from 10 to 500 text messages daily across all users regarding Customer Care, Account Information and Marketing. Individual message frequency depends on your appointment schedule, account activity, and communication preferences.
Standard Messaging Disclosures
- Message and data rates may apply. Standard messaging rates from your mobile carrier apply to all messages sent and received. Contact your wireless provider for details about your messaging plan.
- Reply STOP to any message at any time to opt out of all SMS notifications.
- Reply START to re-subscribe to SMS notifications after opting out.
- Reply HELP for help and support information.

How to Opt Out
Reply STOP to any message at any time to immediately opt out of all SMS notifications from Be Well LifeStyle Centers.
After sending STOP, you will receive one final confirmation message and no further SMS messages will be sent unless you re-opt-in by texting START.

Full Terms of Service
By opting in to receive SMS messages from Be Well LifeStyle Centers, you agree to the following terms:
- Consent to Receive Messages: You consent to receive autodialed and/or pre-written SMS/MMS messages from Be Well LifeStyle Centers at the mobile number you provided. This includes messages sent using an automatic telephone dialing system.
- Recurring Messages: You understand that messages are recurring and may be sent on an ongoing basis until you opt out.
- No Purchase Required: Consent to receive SMS messages is not required as a condition of purchasing any goods or services from Be Well LifeStyle Centers. You may opt out at any time and still receive our healthcare services.
- Access by Others: You acknowledge that SMS messages may be accessible by anyone with access to your mobile device. Be Well LifeStyle Centers is not responsible for messages viewed by unintended recipients.
- Accuracy of Information: You represent that the mobile phone number you provide is your own and that you are authorized to receive SMS messages at that number.
- Limitation of Liability: Be Well LifeStyle Centers, its affiliates, and any third-party service providers (including but not limited to Twilio) shall not be liable for any delays, failures of delivery, or errors in the content of SMS messages. To the maximum extent permitted by applicable law, in no event shall Be Well LifeStyle Centers be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or in connection with the SMS messaging program.
- Indemnification: You agree to indemnify, defend, and hold harmless Be Well LifeStyle Centers and its officers, directors, employees, and agents from any claims, liabilities, damages, or expenses arising from your participation in the SMS messaging program or your breach of these Terms of Service.
- Modification of Terms: Be Well LifeStyle Centers reserves the right to modify these Terms of Service at any time. Changes will be posted to this page with an updated effective date. Your continued participation in the SMS messaging program after changes constitutes your acceptance of the modified terms.
- Governing Law: These Terms of Service shall be governed by and construed in accordance with the laws of the State of Michigan, without regard to its conflict of law provisions. Any disputes arising under these terms shall be resolved in the courts of the State of Michigan.
- Severability: If any provision of these Terms of Service is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
